4Napoleon McCallum
It was the opening game versus the San Francisco 49ers in 1994. The LA Raiders running back Napoleon McCallum rushed up the middle and got tackled by a group of defenders. He fell backward, but his leg was caught between the players and it resulted in his kneecap dislocation. Sadly that was the last game he ever played, but at least as a consolation, he did manage to score a touchdown prior to the injury.
